Learning Objectives: 

1) Develop an understanding of health literacy, how it can be assessed, how it can affect decision making, and ways in which communication can be improved in patients with lower health literacy. Learn about how patients with mental illness may be supported in AKC to their improve outcomes. 

2) Learn about the latest developments and considerations in vascular access to get the right access for the patient at the right time. Develop an understanding of indications to start dialysis and communication with patients. 

3) Consider strategies for increasing uptake of home haemodialysis and transplantation in older patients. Develop an understanding of which patients may benefit and optimizing chances of long-term success.
