​NHS dietitians: if your course fees are paid by your Trust, please make sure you have a purchase order number before completing this application form. 

SELF-FUNDED – Payment by Card 
If you are self-funding your place, please choose the Self-funded – Payment by Card category. If you need a VAT receipt, please email estore@kcl.ac.uk after you have made your booking. Please note that invoices cannot be issued for card payments.  

SPONSORSHIP - Purchase Order Number 
If your place of work (e.g. NHS Trust or other hospital) is paying for your place on the course, please make sure you have a Purchase Order (PO) number ready before starting the application form. A PDF copy of the PO must be emailed to fodmaps@kcl.ac.uk. Your application will not be processed until we receive a copy of the PO.

NON-UK DIETITIANS 
For international dietitians, please send evidence of your registration with the national dietetic association in your country to fodmaps@kcl.ac.uk to obtain dietitian’s recognition approval before applying to the course. We will let you know if you qualify shortly afterwards. ​

Refund Policy:

If you have not accessed the course via the relevant virtual learning environment or other online platform, the course fees will be reimbursed in full. If you have accessed the Course already (regardless of whether you have viewed or completed any section yet), you shall not be entitled to a refund of any course fees.
