Important Application Information 

Please ensure you have all the required details with you before you start the questionnaire, as you won’t be able to save it and return to it at a later stage. Please note that you have about 10 minutes to complete the application once started. 

Please note that the application should be completed by the applicant themselves.  

SELF-FUNDED – Payment by Card
If you are self-funding your place, please choose the Self-funded – Payment by Card category. If you need a VAT receipt, please email estore@kcl.ac.uk after you have made your booking. Please note that invoices cannot be issued for card payments.  

SPONSORSHIP - Purchase Order Number 
If your place of work (e.g. NHS Trust or other hospital) is paying for your place on the course, please make sure you have a purchase order number ready before starting the application form. Without a purchase order number, your application will be rejected. 

NON-UK DIETITIANS 
For international dietitians, please send evidence of your registration with the national dietetic association in your country to fodmaps@kcl.ac.uk to obtain dietitian’s recognition approval before applying to the course. We will let you know if you qualify shortly afterwards. 

FODMAP BOOKLETS
One copy of the Reducing and Suitable booklets will be sent to you by post, as part of the course materials. They will be sent to the personal address provided on the application form. Please make sure the address is correct and in full.

Refund Policy

The fee will be refunded in full only for cancellations received more than 42 days before the short course start date; partial refund can be arranged for cancellations received more than 8 days before the short course start date.
