Detailed Description
Physiotherapy alumni students can request a Canadian Alliance of Physiotherapy Regulators (CAPR forms) to be completed to confirm placement details, such as location, dates, areas of practice and hours.
The application form(s) is to be emailed directly to the Physiotherapy administrative team at [email protected] in order for the Programme Officer to complete.
CAPR is responsible for assessing the educational credentials and qualifications of individuals who wish to practice physiotherapy in Canada. CAPR works on behalf of Canadian physiotherapy regulators.
The documents CAPR require are the following:
- Form D along with a supervised clinical practice document that supports/confirms the supervised clinical practice hours provided by the institution on page 5 of Form D. This document must provide the locations, start and end dates, areas of practice, and hours of each of the supervised clinical placements the applicant completed for their program. As this document is meant to support/confirm the information on form D, the hours must align with those on Form D. Institutions can also use the CAPR template to verify this information.
- A sealed/stamped transcript is also required for the completion of the CAPR forms. These are to be ordered separately via the following eStore link: Academic Transcripts. The sealed transcript is to come directly to Education Support Services department, attention of the Physiotherapy Programme Officer at:

- Forms/documentation must be completed by the institution itself – applicants are not to complete the forms. Each document is required to have an original and official institution stamp and handwritten signature. KCL will then send these documents directly to CAPR in an envelope that is stamped and sealed.
CAPR does not require a syllabus or Programme Handbooks. Only the forms listed will be completed in the CAPR request. No additional forms will be completed.
